Hui Mālama Ola Nā ʻŌiwi is currently recruiting for an Outreach Worker to provide direct services to Native Hawaiians and Hawaii Island residents addressing COVID-19 and other health care needs. The Outreach Worker will invariably reach out to clients who may have limited access to health care and shall provide access to services provided by Hui Mālama Ola Nā ʻŌiwi and other community agencies. This position entails intake, screening, and follow-up activities with clients in primary health care, health promotion, and risk reduction services. This position requires travel to and from various locations on Hawaii Island and may be required to work evenings and on weekends to meet the needs of the agency. This is funded under the American Rescue Plan Act (ARPA) and is subject to end on July 31, 2023.
Duties and Responsibilities
Under the direction of the Program Manager, Health Education & Outreach:
- Conducts initial registration and intake data collection through interview in accordance with standard client enrollment procedures
- Assists clients with identifying specific health and medical concerns and areas of need
- Takes vital signs and routinely monitor height, weight, blood pressure
- Assists with assessing clients' medical insurance status, needs, and ability to pay for services
- Assists clients with completing application forms for medical insurance
- Provides support with community COVID-19 testing and vaccination efforts
- Provides education on mitigating the spread of COVID-19
- Provides service provider contact information and appointment information
- Assists with scheduling transportation to medically-related appointments, medication and medical supplies pick-up
- Assists with referrals to community agencies
- Assists with providing intervention for urgent needs or emergencies
- Provides follow-up care to services provided within required timelines
- Reports changes to Program Manager, Health Education & Outreach in client's status and condition as they occur accordingly; documents services and treatment in clients' charts and on the Health Risk Assessment and Plan
- Serves as client advocate when requested/required, and participates in client case conferences
- Interprets program goals, policies and procedures to clients, providers, and the community
- Checks and verifies client information for accuracy, and enters into client records (electronically in eCW
- and on hard copy)
- Updates and maintains case records under the supervision of the Program Manager, Health Education & Outreach
- Attends meetings and training sessions as required
- Assists with Health Enhancement Program Services as needed
- Performs other duties as assigned
Knowledge and Abilities
- Ability to communicate appropriately with clients and their families in a manner that is culturally sensitive and respectful, including use of discretion in keeping client information in strict confidence
- Ability to establish and maintain favorable and comfortable professional relationships with clients, agency staff, and other networking providers at all times
- Ability to work effectively with people from diverse backgrounds and professions
- Ability to read, write and communicate verbally in an appropriate manner with respect to the client's ability to communicate
- Must be able to maintain composure and professionalism at all times
- Ability to complete forms, input data into the computer, and enter information into client charts with accuracy
- Utilize appropriate grammar and accurate medical terms
- Ability to maintain the confidentiality of client information in accordance with HIPAA compliance
- Ability to sit and work at a computer for extended periods of time
- Must be able to lift and carry 40 lbs
- Must be able to work in a variety of settings which include indoor, outdoor and public areas
- Must be highly self-directed and able to work independently
Minimum Qualifications
- Education
- Graduation from an accredited school or university with an Associate's Degree (AS) in a health or human service field, social work, public health
- or High School Diploma or General Education Degree with a minimum of 5 years of working experience
- Graduation from an accredited school or university with an Associate's Degree (AS) in a health or human service field, social work, public health
- Experience
- Three (3) years of experience in the related area.
- Other
- Proficient use of office equipment such as PC, laptop, tablet, Chromebook, fax machine, scanner, printer, and associated software applications. Familiar with Microsoft Office Systems, to include Word, Excel, Publisher, Power Point, Google platform, and EHC (electronic health records); experience in eClinicalWorks is a plus
- Must be detail oriented with strong organizational and time management skills and demonstrated ability to multitask effectively in a fast-paced environment within resource constraints and deadlines
- Must possess exceptional interpersonal and collaboration skills

- Current TB Clearance (2 step)

- Bloodborne Pathogens Training
- CPR/AED/First Aid Training
- Personal reliable vehicle with current insurance & registration
- Current Hawaii Drivers License
