Hui Mālama Ola Nā ʻŌiwi is recruiting for a Community Health Worker to assist patients in navigating the healthcare system and to provide culturally appropriate health education on topics related to chronic disease prevention, physical activity, and nutrition. Their goal is to improve patients' health knowledge and self-sufficiency by helping them understand their health condition(s) and develop strategies to improve their health and wellbeing. Under indirect supervision, works closely with primary care teams, behavioral health specialists, and health educators. Works in both clinical and community-based settings, including client's homes.

Duties and Responsibilities

  • Assists clients in their homes, community, or clinic setting. Communicates to clients/patients the purposes of the program and the impact it may have on their wellbeing. Helps patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
  • Documents all client encounters and contracts made on behalf of clients; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client. Documents activities, service plans, and outcomes achieved by client in an effective manner.
  • Coaches patients in effective management of their chronic health conditions and self-care. Assists patient in understanding care plans and instructions. Motivates patients/clients to be active and engaged participants in their health and overall wellbeing.
  • Assists clients in accessing health related services, including but not limited to obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and /or social services.
  • Provides support and advocacy during initial medical visit or when necessary to assure clients' medical needs and referrals required are being conveyed. Follows up with both clients and providers regarding health/social services plans.
  • Continuously expands knowledge and understanding of community resources and services. Facilitates client access to community resources, including locating housing, food, clothing, prenatal classes, parenting, and providers to teach life skills, and relevant mental health services. Assists clients in utilizing community services, including scheduling appointments with social services agencies and assisting with completion of applications for programs for which they may be eligible.
  • Facilitates communication and coordinate services between providers and the clients/patients. Coordinates and monitors services, including comprehensive tracking of clients' compliance in relation to care plan objectives.
  • Works collaboratively and effectively within a team. Establishes positive, supportive relationships with participants and provides feedback to other members of the team. Builds and maintains positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds. Works to reduce cultural and socio-economic barriers between clients and institutions.
  • Travels extensively to client homes, community locations, various agencies, and other outreach destinations.
  • Provides education and support on lifestyle modifications (e.g., healthy diet, the importance of exercise). Teaches concepts of health promotion and disease prevention.
  • Communicates and collaborates with clients and families to identify concerns and remove barriers that affect service delivery or client satisfaction.
  • Builds partnerships with individuals, families, youth, and key stakeholders to promote recovery, resiliency, and wellness. Promotes the development and use of member and family support programs.
  • Other duties as assigned.

Knowledge and Abilities Required

  • Strong interpersonal and communication skills and the ability and knowledge to work effectively with a wide range of community agencies and resources.
  • Organizational skills with the ability to multitask and accuracy with strong attention to detail.
  • Working knowledge of outreach programs related to health care delivery, clinical education, and health-related service.
  • Ability to communicate medical information to health care professionals and care coordinators over the telephone.
  • Creative and analytical thinking and the ability to plan, implement and evaluate individual client care plans.
  • Ability to work independently and in a team setting and able to multitask in a fast-paced environment.
  • Proficient use of office equipment such as PC, laptop, tablet, Chromebook, fax machine, scanner, printer, and associated software applications to include electronic health record.
  • Must have a reliable vehicle.
  • Valid Hawaiʻi driver's license

Minimum Qualifications

  • Education
    • GED or a high school diploma plus successful completion of a community college community health worker program.
  • Experience
    • Three years of general experience plus one year of experience directly related to the social work setting, relevant area, or duties and responsibilities specified.
    • Associates degree in a relevant field may substitute experience requirement.
  • Other Certifications and Requirements
    • Community Health Worker Certificate (required within 18 months of employment)
    • Current TB clearance (2 step)
    • HIPAA Compliance Training
    • Bloodborne Pathogens Training
    • CPR/AED/First Aid Training
    • Personal reliable vehicle with current insurance & registration
    • COVID-19 vaccination*

Desirable Qualifications:

  • Familiar with HMONO's mission, programs, and services.
  • Possess a working knowledge of Native Hawaiian culture, traditions, history, and current events including, but not limited to State and Federally-funded programs and projects. Keep abreast of Native Hawaiian issues and current events.
